SHANTHI MANDIR Facilities Use AGREEMENT EFFECTIVE October 1, 2005
 

1. IMPORTANT NOTE: A key will be required to enter the building. There is a $100 refundable deposit required to obtain the key to use the temple building for a private event. The deposit may be refunded no sooner than 3 business days after the event.

 Key pickup can be arranged by contacting Inder Khurana at 882-3474. If Inder can’t be reached, then key pick-up can be arranged with H. R. Chandrasekhar at 446-6143 or  V. S. Gopalaratnam at 446-0663.

 The key must be returned the next business day by NOON following the event to ensure refund of your deposit. We will retain $50 of your deposit fee if the key is NOT returned the next business day.

 2.Facilities use donation is not refundable if you are not able to use the facility on the scheduled day for any reason. However rescheduling will be permitted to allow use of the facility on another day/time when the facility is available.

 3. User has 30 minutes prior to their reservation time AND 30 minutes after their reservation time for set up and clean up. Any time used outside of the reserved time may incur additional charges.

 4. The user shall restore the facility to the conditions existing prior to use.

 It shall be the responsibility of the user to provide all their own decorating supplies, provide for their own set-up and cleaning, clear the facility area of all trash and place all trash in outside trash can near the tool shed and remove supplies brought in by the user. No decorations shall be affixed in any manner to painted walls.

 No smoking is allowed inside the building. Vegetarian Food and beverages are permitted throughout the facility. Kitchen is available for use. User is responsible for ALL cleanup.

 No weapons, even those held by a concealed weapons permit, are allowed in the premises.

 Guidelines for cleaning are posted in the building. Following the use of the facilities, should the condition of the building fail to meet these guidelines, a fee of $25 per hour (a minimum charge of $50 for 2 hours) for cleaning will assessed.

 5. Tables and chairs are to be used indoors only and only for the specific purpose for which they are intended.

 6. Maximum group size for this location is 100 persons. Chaperones will maintain order at the event. Minimum chaperone requirements are as follows: 1 adult over the age of 21 per 10 youth, 1 adult over the age of 25 per 15 teens.

 Order must be maintained by the organization or person(s) using the facility, both inside the facility and on the grounds. Unusual rowdiness or abuse of persons or property, if not managed by the user, could result in immediate termination of the agreement.

 7. For any Pooja or religious event, all foot wear needs to be left in the corridor near the front door, outside the prayer hall.

 8. Any lamps lit for religious occasions have to be under the supervision of an adult. Extra care must be exercised during “aarti” to ensure fire safety. All open flames must be extinguished before leaving the facility.

 9. Alcohol and non-vegetarian food is not permitted on the premises.

 10. Amplified music is permitted but must be kept to a reasonable level and a sound permit is required from the police department.

 11. Persons using the facility are responsible for complying with all city, county, state and facility regulations, including state liquor laws. NO sale or use of alcohol/drugs is permitted. 

 The Organization owning the property carries no medical insurance and assumes no liability for personal injuries or loss of personal property on persons participating in these activities. A participant's family policy must cover any medical costs incurred.
